Superior Wolven gear (Superior Wolven). You wear it from level 29. Crafting uses up an enhanced piece.
Combat style
The Wolven set is a medium hybrid. Pieces give you attack and adrenaline, so you mix fast and strong slashes and keep the pace in a fight. When you want only fast attacks, pick the Cat set. This set is a mix of everything, signs included. It fits Aard, Igni, Quen, Yrden, and Axii.

Where to find the diagrams
For the superior Wolven set you need two Hieronymus notes. You buy one from the armorer in Kaer Trolde and the other from the armorer on Hierarch Square in Novigrad. After you read them, objectives pop up on Skellige and in Kaer Morhen.
Pick up the chest in ruins southwest of Fyresdal, where harpies nest. Gloves sit in the iron mine south of Kaer Morhen. Trousers lie in a cave north of the Bastion. Boots are in a cave east of the river by the fortress.
The steel sword is on Hindarsfjall, southwest of Lofoten. The silver sword lies by stone circles southeast of Arinbjorn.

Who crafts it
Most pieces are forged by an apprentice armorer or blacksmith. The chest needs a master armorer, usually Yoana at Crow’s Perch, and you have the list in the blacksmiths and armorers overview.
